When Internet Nursery is applicable, parent(s) understand and agree to the following:

• Infant's photo will only be taken immediately prior to discharge of mother and infant;
• Photo will be available on the internet for anyone with internet capabilities to see;
• Parents' names, infant's photo, first name, date of birth, weight, length and time of birth will appear on website;
• Photos are posted to website two times per week and may not be immediately available for access upon discharge.
