An individual’s personal information may have been listed in this database if he or she used the online program-registration component of our website, called into Saint Mary’s Telehealth or if he or she participated in any of the following programs or services:

• Community classes (such as CPR or prenatal education)
• Health-and-wellness programs (such as health fairs, immunizations or executive physicals)
• An employer-sponsored health fair that was provided by Saint Mary’s (Example: Company A may have held a health fair for its employees and used Saint Mary’s to provide the health screenings. Data collected from the health fair would have been entered into the affected database.)
• Saint Mary’s Nurse Triage Program (closed three years ago)
• Saint Mary’s Silver Connection Program (closed more than five years ago)
